What to Bring to Your First Visit

To ensure a seamless check-in process and allow us to provide the best care possible, please bring the following:

• Photo ID (e.g., driver's license or state-issued ID)

• Insurance card(s) (including primary and secondary, if applicable)

• Referral form (if required by your insurance)

• Co-pay or deductible payment (cash, credit/debit card, or HSA)

• List of current medications (including dosage and frequency)

• Any relevant medical records or imaging (X-rays, MRI reports, lab work)

• Footwear or orthotics you use regularly (for gait or biomechanical evaluation)
